Important Dates to Remember
In the event that you experience any of the qualifying life events listed below, you should notify your Human Resource department immediately. Under these circumstances you may be eligible to change your Benefit coverage. Typically, employers allow for 30 days from the date of the event to make any changes; however, these dates vary from employer to employer, so be sure to check with your employer. Documentation may be required (i.e. copy of marriage license, birth certificate). Note: depending upon the change, you may want to change your beneficiaries as well.
Your 65th Birthday
Many times you can continue to be primary on your employer's plan. Nevertheless, you should call Medicare and apply for part A coverage and consider applying for the Contributory Part B coverage. Contact us for further details.
Your Wedding Day
Now there are two of you. Remember to have your coverage changed to a family contract.
The Day of Divorce
Check with your plan administrator to make sure of separate "Him" and "Her" contracts...with one of them covering the kids (the one whose birthday comes first in the year). Also, make sure all COBRA notice obligations are satisfied.
The Day You Move
Notify human resources (or your employer) immediately to change your address.
The Day You Go On Vacation
Be sure to take your identification card along.
If You Leave Employment
Federal Legislation (COBRA) may give you the opportunity to continue current coverage at your own expense. See your employee benefits manager for details.
A New Baby
Let's not forget the little one. Be sure your newborn is added to the plan. Recent HIPPA laws can create significant coverage problems for those new babies not added to your plan within 30 days of the birth.
Cost Containment Program
Under your benefits plan, it might be necessary for you to pre-certify and get a second opinion before your admission. Make sure you fulfill these responsibilities in order to avoid reduced benefits.
